Evergreen Adult Day Care is designed for adults who need a little extra help, support, and companionship during the day.
Our program offers structure, social engagement, reliable care, and peace of mind for participants (and their families). Individuals enjoy fun activities, nutritious meals, free transportation, and personalized assistance in a warm, friendly environment.
How the Program Works
Getting started is easy. Anyone interested in joining our day program can stop by for an on-site assessment or call to schedule an appointment with our team to complete the required paperwork.
We’ll guide you through each step, answer your questions, and help you determine if the program is a good fit.
Cost and Payment Options
We are committed to making our services as accessible as possible. Evergreen Adult Day Care accepts two payment types.
Medicaid Waiver
Participants may qualify for financial assistance depending on income. Our team helps walk families through the income qualification process from start to finish.
Private Pay
For those whose income exceeds Medicaid guidelines, private pay options are available.
If you’re unsure what you qualify for, don’t worry. We’ll walk you through the eligibility process.

Daily Support at Evergreen
At Evergreen Adult Day Care, we provide the services participants (and their families) need to feel truly supported. Our offerings include:
- Free Transportation, including daily pick-up and drop-off, and transportation to all medical and therapy appointments.
- Nutritious Meals, including two hot meals daily (breakfast and lunch), as well as an afternoon snack.
- Health Support & Medication Management provided by a Registered Nurse (RN) and certified Med Tech, including daily administration, refill coordination, assistance with pill planners – and we’ll even fill them up prior to the weekends!
- Personal Care Assistance, including bathing help, personal laundry support, and hygiene assistance.
Joy, Connection, and Fun Every Day
Our activities are designed to spark joy, encourage socialization, and keep minds and bodies active – all in a supportive, uplifting environment. Although each day is different, typical activities include:
- Breakfast and lunch outings
- Bingo, cards, and other friendly competitions
- Crafts, baking, and cooking projects
- Afternoon movies with popcorn
- Bowling trips and other indoor games
- Picnics and seasonal celebrations
- Shopping trips to local stores and markets
- Scenic rides and out-of-town excursions
- Social time, conversation, and group gatherings
- Invitations to evening events at The Kensington
